RESUMEN El presente texto es el resultado de un estudio dirigido a demostrar la importancia del sistema muscular en mujeres operadas de cáncer de mama, enfermedad con elevada incidencia en Cuba y el mundo. Se investigó en los principales reportes anatómicos. Se partió de la revisión documental de textos básicos para la carrera de Medicina inherentes a la asignatura Anatomía Humana. Se corroboró que autores como M. Prives, N Lisenkov y V Bushkovich y R. D. Sinelnikov; describen los músculos, pero (...) no relatan su importancia en la práctica quirúrgica. Se emplearon los buscadores en Internet y otros sitios webs de información científica, que permitieron corroborar desde un enfoque de ciencia, tecnología y sociedad la importancia del tejido muscular y su aplicación en técnicas reconstructivas en pacientes operadas de cáncer de mama. Se constató que los músculos más usados son los rectos abdominales, los dorsales anchos, los gráciles y el tejido adiposo del pliegue glúteo. ABSTRACT The present text is the result of a study directed to demonstrate the importance of the muscular system in women who had a surgery on breast cancer, illness with high incidence in Cuba and the world. It was investigated in the principal anatomical reports. It began with the documentary review of basic texts for the career of Medicine inherent to the subject Human Anatomy. It was corroborated that authors like M. Prives, N Lisenkov and V Bushkovich and R. D. Sinelnikov; they describe the muscles, but they do not report their importance in surgical practice. It was used the searche engines on Internet and other web sites of scientific information, which allowed to corroborate from an approach of science, technology and society the importance of the muscular tissue andits application in reconstructive techniques in patients who undergoa breast cancer surgery. It was stated that the most used muscles are the abdominal rectus, the latissimusdorsi, the graceful ones and the adipose tissue of the gluteus fold. (shrink)

Food choices are a key determinant of dietary intake, with brain regions, such as the mesolimbic and prefrontal cortex maturing at differential rates into adulthood. More needs to be understood about developmental changes in healthy and unhealthy food perceptions and preference. We investigated how food perceptions and preference vary as a function of age and how food attributes impact age-related changes. One hundred thirty-nine participants completed computerized tasks to rate high-calorie and low-calorie food cues for taste, health, and liking, followed (...) by 100 binary food choices based on each participant’s ratings. Dietary self-control was considered successful when the healthier food was chosen. Self-control success ratio was the proportion of success trials over total number of choices. Beta-weights for health and taste were calculated as each attribute’s influence on food preference. Adiposity measurements included BMI z-score and waist-to-height ratio. High-calorie foods were rated more tasty and less healthy with increasing age. Older participants liked high-calorie foods more, and β-taste was associated with age. Significant age-by-WHtR interactions were observed for health and taste ratings of high-calorie foods, β-taste, and marginally for preference of high-calorie foods. Stratifying by WHtR, we found age-related increases in taste and preference ratings of high-calorie foods in the high WHtR group alone. In contrast, age-related decreases in health ratings of high-calorie foods were significant in the low WHtR group alone. Age and β-taste were significantly associated in the high WHtR group and only marginally significant with low WHtR. Although participants rated low-calorie foods as less tasty and less healthy with increasing age, there was no association between age and preference for low-calorie foods. Participants made faster food choices with increasing age regardless of WHtR, with a significant age-by-WHtR interaction on reaction time. There were no age-related effects in self-control success ratio and β-health. These results suggest that individual differences in age and central adiposity play an important role in preference for high-calorie foods, and a higher importance of food tastiness in food choice may contribute to greater preference for high-calorie foods with increasing age. A prefrontal control system that is less mature than the limbic reward system in adolescence is thought to impede self-regulatory abilities, which could contribute to poor dietary choices and obesity. We, therefore, aimed to examine whether structural morphology of the prefrontal cortex and the amygdala are associated with dietary decisions and obesity in children and adolescents. Seventy-one individuals between the ages of 8–22 years participated in this study; each participant completed a computer-based food choice task and a T1- and T2-weighted (...) structural brain scans. Two indices of obesity were assessed, including age- and sex-specific body mass index and waist-to-height ratio. The behavioral task included rating 60 food stimuli for tastiness, healthiness, and liking. Based on each participant’s self-ratings, 100 binary food choices were then made utilizing a computer mouse. Dietary “self-control” was calculated as the proportion of trials where the individual chose the healthier food item over the total number of trials. Cortical thickness and amygdala subnuclei volumes were quantified using FreeSurfer 6.0 and CIT168 atlas, respectively. We found that WHtR was negatively associated with the thickness of bilateral superior frontal, left superior temporal, right insula, and right inferior temporal regions. We also found WHtR to be positively associated with the volume of the central nucleus region of the amygdala, after adjusting for the hemisphere, age, sex, and intracranial volumes. A similar data pattern was observed when BMIz was used. Moreover, we found that across all participants, thinner right superior frontal cortex and larger left CEN volumes predicted lower dietary self-control. These results suggest that differential development of the PFC and amygdala relate to obesity and dietary self-control. Further longitudinal studies are merited to determine causal relationships among altered PFC to amygdala neural circuitry, dietary self-control, and obesity. (shrink)

Se propone una reflexión sobre el método clínico y sus etapas, la relación médico-paciente y la presencia tecnológica en la Cardiología Pediátrica. La ecocardiografía transtorácica no puede sustituir el pensamiento médico, la anamnesis, ni el examen físico. Como medio diagnóstico realizado e interpretado por humanos, está sujeto a error, el cual se minimiza según la calidad de los datos clínicos aportados por el médico que lo solicita. El presente trabajo tiene como objetivo analizar los aspectos éticos del método clínico en (...) relación con la utilización de la ecocardiografía transtorácica en niños con cardiopatías congénitas. It is proposed a reflexion about the clinical method and its stages, the doctor-patient relation and the presence of technology in paediatric cardiology. The transthoracic echocardiography can4t substitute medical thought, neither the anamnesis nor the physical examination. As a diagnostic medium carried out and interpreted by humans, it is subject to error, which it is minimized according to the quality of the clinical data provided by the doctor who requests them. The present work has the objective to analyse the Ethical foundations of the clinical method in connection with the use of transthoracic echocardiography in children with congenital cardiopathies. Despite the shifting ideologies of gender of the seventeenth century, the arrival of the first actresses caused deep social anxiety: theatre gave women a voice to air grievances and to contest, through their own bodies, traditional gender roles. This paper studies two of the best-known actresses, Nell Gwyn and Anne Bracegirdle, and the different public personae they created to negotiate their presence in this all-male world. In spite of their differing strategies, both women gained fame and profit in the male-dominated (...) theatrical marketplace, confirming them as the ultimate “gender benders,” who appropriated the male role of family’s supporter and bread-winner. Es escasa la investigación sobre las competencias necesarias para educar eficazmente en los primeros años de vida, y sobre los factores que influyen en la adquisición de una identidad docente de calidad para trabajar en Educación Infantil (EI). Los resultados de la presente revisión integradora apuntan que (1) el profesorado excelente de EI atiende a la diversidad de su alumnado, (2) las comunidades de práctica contribuyen a la adquisición de una identidad docente excelente, y (3) ciertas deficiencias en los programas (...) de formación docente y determinados factores contextuales obstaculizan un desempeño docente excelente. En este libro (de 589 págs.) se muestra cómo Ortega descubrió la vía o método de la razón vital, que permite comprender la realidad. Gracias a este método de la razón vital, Julián Marías ha llegado a una nueva concepción de la filosofía y del cristianismo. Y por ello ha llegado a ser el "apóstol de la divina razón" justificadora tanto del cristianismo como de ese quehacer que llamamos filosofía, la cual es tan necesaria para él que sin ella nos (...) quedamos privados de los instrumentos intelectuales adecuados para comprender a Dios. Expone lo más fielmente posible la divina razón filosófica y teológica de Julián Marías, dando cuenta de su dilatada obra, dejando que él nos “hable” con los textos que escribió. El presente estudio atiende a un deseo expresado por él mismo de poner sus escritos en conexión: «cuando he publicado un libro, se ha hablado de él casi siempre como si fuese “único”, sin intentar casi nunca ponerlo en conexión con los anteriores, menos aún intentar hacer la suma. Es un carácter que valdría la pena examinar». Al poner en conexión su densa obra, este libro busca principalmente sistematizarla, así como completar con lo que dice en unos libros lo que falta, aparece solo apuntado o está en trance de maduración en otros. Porque, como todo pensador, Julián Marías tiene también sus deficiencias, una de las cuales (la principal) es que no hizo una sistematización rigurosa de su filosofía: esta fue apareciendo progresivamente en el curso de su vida hasta los últimos artículos —unos 250— que dictó al autor del presente libro. No pudo hacer esa tarea por falta de tiempo. Y porque no se daba importancia. Muchos lectores resbalan sobre sus últimos libros, como "Persona" o "La perspectiva cristiana" —brevísimos porque así lo quiso su autor, que modestamente no insistía acerca de todo lo que descubren y de lo que implica cada frase— sin apenas sospechar que nos encontramos ante obras monumentales, realmente innovadoras. Realizar esa sistematización de toda su obra no es tarea fácil. En todo caso, nos damos cuenta de la genialidad de Julián Marías —que además escribe con admirable talento literario, «con su diamantina prosa» —, según la siguiente definición, en un libro suyo de literatura, a propósito de su amigo Azorín: «Un genio es quien modifica nuestra manera de ver la realidad, de tal manera que ya forma parte de nosotros» . Introduction Currently, The nature and scope of Clinical Ethics Protocols in Madrid are not well understood.Objectives The main objective is to describe the features of ‘guideline/recommendation’ type CEPs that have been or are being developed by existing Clinical Ethics Committees in Madrid. Secondary objectives include characterisation of those CECs that have been the most prolific in reference to CEP creation and implementation and identification of any trends in future CEP development.Methods We collected CEPs produced and in process by CECs accredited (...) in the public hospitals in Madrid, Spain, from 1996 to 2008.Results CECs developed 30 CEPs, with 10 more in process. The most common topic is refusal of treatment . If CEPs addressing terminal illness, Do-Not-Resuscitate orders and advance directives are placed into a separate ‘ethical problems at the end of life’ category, this CEP subject emerges as the most common . There is a relationship between the age of the CEC and the development of CEPs . CECs now seem to be more likely to engage in CEP development.Conclusions The CECs in Madrid, Spain, have developed a significant number of CEPs and there is a trend towards continued development. The most frequent topics are ethical problems at the end of life and refusal of treatment by the patient. (shrink)

Resumen Este artículo analiza, en discusión con Joaquín García-Huidobro y Alejandro Miranda, la conveniencia de utilizar en moral sexual el argumento de la facultad pervertida, conforme al cual sería inmoral frustrar el fin natural de las facultades reproductivas. Según García-Huidobro y Miranda este argumento sólo puede utilizarse desde el “principio de totalidad”, pues su uso aislado llevaría a los absurdos denunciados por la New Natural Law Theory. Con vistas a una reconsideración de este argumento, se demuestra la importancia de considerar (...) la ética en su carácter tópico o contextual. Así, es en el ámbito de la templanza y de las inclinaciones sexuales donde el AFP cobra un papel normativo; en cambio, el “principio de totalidad” entra en juego cuando se está considerando el problema del límite de las intervenciones médicas.This paper focuses on the adequacy of using the so-called perverted faculty argument within the context of sexual morality. According to this argument, frustrating the reproductive faculty would be immoral. Professors García-Huidobro and Miranda think that this argument can be appropriately used only within the framework of the principle of totality. They argue that an unqualified use of the PFA would lead to the absurdities pointed out by the New Natural Law Theory. In order to see the PFA in a new light we demonstrate the importance of respecting the dialectical or contextual nature of Ethics. Thus, regarding temperance, and especially sexual inclinations, the PFA has a clear normative role. The principle of totality, instead, must be used when considering the problem of the moral limits of medical interventions. (shrink)

In this article, we present results from a literature review of intrinsic, instrumental, and relational values of nature conducted for the Intergovernmental Science-Policy Platform on Biodiversity and Ecosystem Services, as part of the Methodological Assessment of the Diverse Values and Valuations of Nature. We identify the most frequently recurring meanings in the heterogeneous use of different value types and their association with worldviews and other key concepts. From frequent uses, we determine a core meaning for each value type, which is (...) sufficiently inclusive to serve as an umbrella over different understandings in the literature and specific enough to help highlight its difference from the other types of values. Finally, we discuss convergences, overlapping areas, and fuzzy boundaries between different value types to facilitate dialogue, reduce misunderstandings, and improve the methods for valuation of nature's contributions to people, including ecosystem services, to inform policy and direct future research. © 2023 The Author(s). This paper investigates the role that universities play in supporting social entrepreneurs (SEs) across their ecosystem. Adopting the resource-based view (RBV) approach, we argue that universities attract, mobilize, and deploy multiple resources that benefit SEs through four main mechanisms (i.e., teaching, research, outreach, and the development of partnerships). We use a qualitative approach of 62 semi-structured interviews and 8 focus groups in Ecuador, Bolivia, and Colombia. Our contribution shows that employing different resources and engaging in supportive activities of universities towards (...) SEs facilitate the development of university capabilities, such as an increased reputation, development of leadership skills for students and staff, and wider access to ecosystem resources that support other university activities related to teaching, research, and community outreach. The role that partnerships play in the development of these capabilities is fundamental, thanks to the different strategic alliances and mobilization of resources between universities and other actors while supporting SEs. This study contributes to our understanding of the role universities can play to intensify the nature of supportive ecosystems for SEs. This contribution is relevant not only to academics but also to practitioners, as this will shed light on the building, development, and scaling of a supportive ecosystem for SEs. (shrink)

John Locke's philosophy was deeply affected by medicine of his times. It was specially influenced by the medical thought and practice of Thomas Sydenham. Locke was a personal friend of Sydenham, expressed an avid interest in his work and shared his views and methods. The influence of Sydenham's medicine can be seen in the following areas of Locke's philosophy: his “plain historical method”; the emphasis on observation and sensory experience instead of seeking the essence of things; the rejection of hypotheses (...) and principles; the refusal of research into final causes and inner mechanisms; the ideal of irrefutable evidence and skepticism on the possibilities of certainty in science. The science which for Locke held the highest paradigmatic value in his theory of knowledge was precisely medicine. To a great extent, Locke's Essay on Human Understanding can be understood as an attempt to justify, substantiate, and promote Sydenham's medical method. This method, generalized, was then proposed as an instrument for the elaboration of all natural sciences. (shrink)

Synaesthesia is a condition in which one type of stimulation evokes the sensation of another, as when the hearing of a sound produces photisms, i.e. mental percepts of colours. R is a 20 year old colour blind subject who, in addition to the relatively common grapheme-colour synaesthesia, presents a rarely reported cross modal perception in which a variety of visual stimuli elicit aura-like percepts of colour. In R, photisms seem to be closely related to the affective valence of stimuli and (...) typically bring out a consistent pattern of emotional responses. The present case study suggests that colours might be an intrinsic category of the human brain. We developed an empirical methodology that allowed us to study the subject's otherwise inaccessible phenomenological experience. First, we found that R shows a Stroop effect elicited by photisms despite the fact that he does not show a regular Stroop with real colours. Secondly, by manipulating the colour context we confirmed that colours can alter R's emotional evaluation of the stimuli. Furthermore, we demonstrated that R's auras may actually lead to a partially inverted emotional spectrum where certain stimuli bring out emotional reactions opposite to the normal ones. These findings can only be accounted for by considering R's subjective colour experience or qualia. Therefore the present paper defends the view that qualia are a useful scientific concept that can be approached and studied by experimental methods. (shrink)

La pena natural es una situación paradójica que se puede presentar cuando el autor de un delito, al cometerlo o a causa de éste, ha sufrido un daño igual o mayor al que padecería si el juez de la causa le aplicara la pena estatal prevista para tal conducta. Se da con frecuencia al juzgar ciertos accidentes viales, cuando el acusado ya carga el peso de ser quien, por imprudencia, mató a su pareja o familiar cercano, por ejemplo. Pero ¿en (...) qué situaciones y cómo cabe recurrir a la pena natural? En esta investigación, Manuel Serrano explora las diversas aristas y posturas en pugna que atraviesan este tema, donde se cruzan cuestiones como la idea de la “culpa compensada”, la prohibición de aplicar castigo doble por un mismo hecho (non bis in idem) y la clemencia judicial. El autor desmonta la estructura conceptual de la pena natural, estudia jurisprudencia argentina y recorre producciones teóricas de juristas que se han preocupado por este asunto complejo, profundo y poco abordado del derecho penal. El trabajo consiste en una elucidación de los elementos que conforman el concepto de pena natural (poena naturalis) en el Derecho penal. Se puede caracterizar la pena natural como el daño o sufrimiento que recae sobre el autor de un delito, producto de la comisión del mismo, que debe ser descontado de la pena legal que ha de aplicársele. Si bien existe un mínimo acuerdo sobre esto, tanto en la jurisprudencia como en la doctrina penal se observan serios desacuerdos acerca (...) de los requisitos que se deben cumplir para reconocer que se está frente a una pena natural. Para elucidar los elementos, se utilizará el constructivismo conceptual de Ronald Dworkin, a fin de describir el desacuerdo y sus implicancias, reconocer los elementos principales de la pena natural y justificarlos bajo su mejor luz moral. ABSTRACT This study focuses on the clarification of the conceptual elements of poena naturalis in criminal law settings. The poena naturalis is the damage or suffering that falls on the convict for its commission. This damage must be deducted from the poena forensis to be applied. Although there is a minimum agreement about the concept of poena naturalis, in the jurisprudence and among legal scholars, there are serious disagreements about the elements of poena naturalis. To elucidate the elements, I will adopt Ronald Dworkin’s conceptual constructivism to describe the disagreement and its implications. At last, I will justify the main elements of the poena naturalis by their best moral light. (shrink)

¿Qué pasaría si, en el momento fundacional de la modernidad, el personaje principal no fuera el yo cartesiano, sino ese otro personaje que Descartes llamó genio maligno? Esa es la hipótesis que plantea el presente ensayo: ¿qué ocurriría si en lugar de la agotada certeza del yo, el mundo moderno se hubiera asentado más bien sobre la confusión y sobre la trampa que se condensan en la metáfora del genio maligno? Desde esa hipótesis, la mayor parte de las configuraciones filosóficas (...) de eso que llamamos modernidad aparecen transfiguradas, se convierten en meras repeticiones de aquel fraude básico que ocultan. Pero donde tales configuraciones velan ese gesto al repetirlo, el género de terror y los discursos sobre la angustia y la locura parecen haberlo denunciado como grietas de un edificio sin fondo. Este es el viaje apasionante que se propone en este libro, un viaje en el que autores como Poe, Stevenson o Conrad se atreven a proponernos lecturas de Nietzsche, Heidegger o Deleuze y Foucault, en el que personajes como Frankenstein o el doctor Jekyll nos enseñan aspectos inéditos de la filosofía de los siglos modernos, un viaje en el que la posmodernidad se revela como el penúltimo recurso retórico de un personaje ficticio y casi olvidado. Justo antes de la pandemia por COVID-19, la Organización Mundial de la Salud definió unas prioridades de trabajo para la década 2020-2030. Un grupo interdisciplinario de profesionales de la salud reflexiona sobre estas prioridades, determinando unas categorías de análisis y, desde una perspectiva bioética, analiza cada una de ellas, ve su pertinencia, algunos eventos causales, las implicaciones que pueden tener si no son enfrentadas adecuadamente y hace sugerencias sobre la forma de llevarlas a cabo. En esta primera entrega se analiza (...) el primer grupo de prioridades estratégicas: “poblaciones más sanas”. En las conclusiones se destaca la conveniencia de acometer cada una de ellas de una manera colaborativa, gubernamental y privada; de reestructurar el sector salud con base en estas prioridades, aprovechando la experiencia obtenida en la lucha contra el COVID-19. La relación del ser humano con el medio ambiente también debe cambiar, apelando a la responsabilidad humana por el cuidado del planeta. (shrink)

This article argues that the decision by the Columbian high court to totally ban the advertising and promotion of tobacco products is sound and could indeed be applied to other types of harmful products.

This article briefly reviews categorization models in both cognitive psychology and cognitive phonology in order to set the background for a psycholinguistically plausible account of the classification of the allophones involved in category overlaps (i.e., the overlapping areas between phoneme categories) and in the so-called positions of neutralization. In addition, the traditional proposals of both Bloomfieldian phonemics (i.e., phonetic similarity) and the Prague School (i.e., archiphonemes) are discussed and an alternative proposal is offered. The latter claims that phonological theory should (...) be informed by psycholinguistic evidence obtained by experimental methods. This line of research will progressively make cognitive phonology a functional phonological theory consistent with the way speakers classify the sounds of their language and will contribute to theoretical progress in phonological theory. We performed an EEG graph analysis on data from 31 typical readers and 24 dyslexics, recorded while they were engaged in an audiovisual task and during resting-state. The task simulates reading acquisition as participants learned new letter-sound mappings via feedback. EEG data was filtered for the delta, theta, alpha, and beta bands. We computed the Phase Lag Index to provide an estimate of the functional connectivity between all pairs of electrodes per band. Then, networks were constructed using a Minimum Spanning (...) Tree, a unique sub-graph connecting all nodes without loops, aimed at minimizing bias in between groups and conditions comparisons. Both groups showed a comparable accuracy increase during task blocks, indicating that they correctly learned the new associations. The EEG results revealed lower task-specific theta connectivity, and lower theta degree correlation over both rest and task recordings, indicating less network integration in dyslexics compared to typical readers. This pattern suggests a role of theta oscillations in dyslexia and may reflect differences in task engagement between the groups, although robust correlations between MST metrics and performance indices were lacking. The purpose of cognitive models is to make predictive simulations of human behaviour, but this is often done at the aggregate level.
